This could be by patients, medical professionals, NHS staff and managers, NHS contractors and suppliers, and organised criminal gangs.

For example, patients who deliberately evade NHS charges for things like prescriptions, glasses, and dental treatment is estimated to cost the taxpayer £156 million per year.
